The Clinical Case for LANAP

The LANAP protocol has been FDA-cleared since 2004, when the PerioLase MVP-7 first received clearance for the laser-assisted new attachment procedure, supported by human histological research published in a peer-reviewed periodontal journal (510(k) K030290). This is not a new or experimental technology. A second human histological study, also published in a peer-reviewed periodontal journal, led to a second and more expansive FDA clearance in March 2016 (510(k) K151763ve), this time with an outcome claim that no traditional osseous surgical approach can match: True Periodontal Regeneration of the attachment apparatus, specifically new cementum, new periodontal ligament, and new alveolar bone on a previously diseased root surface. More than 20 years of clinical use, peer-reviewed research, and regulatory history support the LANAP protocol.

Primary candidate criteria 

Patients with any of the following are candidates for evaluation: 

  • Periodontal pocket depths of 5mm or greater
  • Radiographic evidence of alveolar bone loss
  • Bleeding on probing or suppuration
  • Tooth mobility
  • A history of periodontal treatment with disease recurrence
  • Peri-implantitis or failing implants (LAPIP protocol)
  • Patients currently declining traditional osseous surgery
  • Patients on anticoagulant therapy where discontinuation is contraindicated
  • Diabetic patients diagnosed with periodontitis, given the well-documented bidirectional relationship between periodontal disease and glycemic control

Clinical Evidence

This radiographic case demonstrates the regenerative potential of the LANAP protocol. Pre-operative probing depths of 10, 13, and 15mm with Class III furcation involvement and mobility. Six years post-treatment, probing depths measured 4, 3, and 6mm with radiographic evidence of significant bone regeneration and restored periodontal attachment.

The protocol uses the PerioLase MVP-7, a free-running pulsed Nd:YAG laser with a wavelength that is highly specific to diseased tissue and pathological bacteria. It selectively removes the infected sulcular epithelium and diseased connective tissue while leaving healthy tissue intact. No scalpel. No sutures. No bone graft materials required.
